We pride ourselves on making all assessments run as smoothly as possible for our clients – which is why we have created a checklist below to assist you and the injured person in the process.
Information for the referrer
- Please provide background documents and instructions at least 10 working days prior to the scheduled appointment date
- Communicate any special needs or accessibility requirements for the assessment
Information for the injured person
- Have a copy of our “what to expect” brochure
- Bring all copies of recent investigations (including x rays, scans and reports)
- Please arrive 15 minutes prior to the assessment
